My first thought is that he may have cognitive dysfunction, but it is possible that there could be an underlying medical issue behind his symptoms. Liver and kidney problems can create a buildup of toxins in the system that can cause similar symptoms. Cancer, especially brain cancer, can cause symptoms of confusion as well. The coughing may or not be related. That could be lung cancer, heart problems, chronic bronchitis, pulmonary hypertension or even emphysema. It would be a good idea to have your vet check him out and at least run some blood work. An x-ray of the chest would be a good idea as well. If all is clear, they can always try him on some medication for cognitive dysfunction.

Your dog, although flawlessly housetrained throughout their lives, may start to have 'accidents'. If you notice cognitive dysfunction in dogs, it can be identified by a number of clinical signs. The diagnosis might be made if your older dog exhibits behavioural changes that have no apparent physical cause. House training issues can be difficult to resolve. Make sure your dog has every opportunity to go by taking them out frequently, and reward good behaviour. Discuss with your vet if medication would be of benefit for your dog.
